Hematoma within the Ligamentum Flavum of the Lumbar Spine : A Case Report

Abstract

|
Study Design: Case report.
Objective: To report a case of an 80-year-old male patient with intra-ligamentous hematoma of the ligamentum flavum in the lumbar spine.
Summary of the Literature Review: Ligamentum flavum hematoma is a rare cause of spinal stenosis.
Materials and Methods: A 80-year-old male patient presented with gait disturbance accompanied by bilateral pain in the buttocks and thighs, which was aggravated 1 month ago without a history of trauma. Magnetic resonance imaging (MRI) showed a heterogeneous signal change inside the ligamentum flavum, with compression of the dura mater at the L4-5 level.
Results: A L4-L5 laminectomy and a partial facetectomy for decompression and resection of the lesion were performed. The lesion in the ligamentum flavum compressing the dura mater on MRI, which was initially neglected as degenerative hypertrophy, was confirmed as hematoma based on the operative and pathologic findings. One month after surgery, the patient¡¯s radiating pain in the lower limb and gait disturbance had improved.
Conclusions: Spine surgeons should be aware of intra-ligamentous hematoma of the ligamentum flavum, which can be neglected as degenerative hypertrophy, as a possible cause of spinal stenosis.
